If you need to set up a doctor's appointment, or you require help with navigating our health care services, call: (888) 321-DOCS (3627).
If you need a prescription filled, or you have questions about your medications, call Northwell Health's Vivo Health Pharmacy: (844) 411-VIVO (8486).
For an ambulance to the hospital of your choice (when medically appropriate)— call 24 hours a day, 7 days a week: (833) 259-CEMS (2367).

My Checklist Along My Health Care Journey
- I fully understand all of my discharge instructions.
- I understand my diagnosis and which symptoms to look out for.
- I clearly understand the name, dosage, side effects, and purpose of all my medications.
- I understand if I need to make changes to my diet and whether I should limit, or
increase, my activity.
- My caregiver and I understand what kind of care I will need when I leave, and what changes I will need to make at home.
- If I need help with transportation arrangements, I will let my medical team know.
- I have a plan to follow up with my health care provider(s).
- I know who to call in case of an emergency.
